Intake Process

  • Contact our office by phone, email or request an appointment in the client portal

  • Once scheduled, intake paperwork will be sent to you via the client portal

  • Complete the intake paperwork and upload your insurance card

Intake paperwork is required for services. The preferred method of intake paperwork completion is the client portal, where paperwork can be signed and filled out online securely before the first appointment. After we receive your request for your initial meeting, we will send you the paperwork. If you have been unable to complete the intake paperwork before the first appointment, the clinician will review the forms with you and have you sign and you will be asked to complete remaining paperwork before the clinician will be able to see you.

 

If using insurance, your insurance information needs to be provided to schedule your appointment. You can upload your insurance information through the client portal, where you complete your paperwork. In the first session, the clinician will conduct an assessment to assign a billing diagnosis. During the evaluation, the clinician will ask you many questions about what is currently happening in your life and developmental questions.
